Requirements for Applying for an Ethiopian Origin Identity Card

1. New Ethiopian Origin Identity Card (Including Those Who Have Previously Held a Yellow Card)

  1. Certified birth certificate from the Ministry of Foreign Affairs of Ethiopia (UK birth certificates must be certified by a UK-authorized document verification agency: www.legalisation@fcdo.gov.uk)
  2. Proof of immediate family relationship (unexpired parent’s Kebele ID, birth certificate, or other proof of relationship)
  3. Copy of the Ethiopian passport (if the applicant was a former Ethiopian citizen)
  4. Copy of current passport
  5. Death certificate certified by the Ministry of Foreign Affairs if the parent is deceased
  6. Proof of education (at least in one of the grades) in Ethiopia
  7. Two 3×4 photographs
  8. Applicant must appear in person
  9. Payment of service fee
  10. If applicant is a minor: certified birth certificate and evidence for parent’s identity
  11. If the applicant is adopted: adoption documents and appearance in person

2. Ethiopian Origin Identity Card Renewal

  1. Applicant must appear in person and fill out the renewal form
  2. Copy of expired Ethiopian National Identity Card
  3. Copy of passport
  4. Two 3×4 photographs
  5. Payment of service fee

3. Lost Ethiopian Origin Identity Card

  1. Police report
  2. Appearance in person
  3. Copy of passport
  4. Two 3×4 photographs
  5. If available, copy or identity number of the lost birth certificate

4. Ethiopian Origin Identity Card Correction

  1. Appearance in person
  2. Copy of identity card
  3. Two 3×4 photographs
  4. Court decision for name changes or certified marriage certificate (if the name change was due to marriage)
  5. Copy of passport (denoting current nationality)
  6. Birth certificate certified by the Ministry of Foreign Affairs of Ethiopia for spelling, date, or place of birth correction

5. Ethiopian Origin Identity Card for Spouse

  1. Certified marriage certificate by the Ministry of Foreign Affairs of Ethiopia
  2. Applicant’s passport
  3. Spouse’s Ethiopian Origin ID (unexpired)
  4. Two 3×4 photographs
  5. Application letter from the Ethiopian-origin spouse
  6. Both applicant and spouse must appear in person

6. Ethiopian Identity Card for Those Under 18 Years

  1. Certified birth certificate from the Ministry of Foreign Affairs of Ethiopia
  2. Copy of parent’s valid Ethiopian identity card
  3. Copy of parent’s passport
  4. Two 3×4 photographs
  5. Applicant’s passport
  6. Copy of child’s valid Ethiopian passport or residence permit if the parent is Ethiopian
